You are now prompted to enter some information about the problem that you wish to report.

First, ensure that the exact URL (web address) of the page where you encountered the issue in question is displayed in the field located directly to the right of the Web site URL label. By default, this field is prepopulated with the address of the current page that you are viewing. This option is not editable, so you must be on the page where the problem exists when attempting to file your report.

If the web page where the problem occurred requires a password to access it, then you should also check the box next to the Web site is password-protected option.
